Yes, they do. They are simply extremely rare. Of the few I have gotten, only one or two have actually been any good. About 10% of the legendaries you get are ancient.

Clear greater rifts, collect Gem of Ease. The drops are random, except the first is Powerful or whatever. From there, level it to 25, cube it to an item to erase the level req. Do another gr, collect another Gem of Ease, repeat.
Is it worth it to reforge legendary Diablo 3?
It's generally cheaper to do so than to reforge legendary items. I've been luckier there and at Kadala than at reforging. If you have lots of forgotten souls and finally have gotten an item that is a must for your build, it might be worth it to reforge the items, but most often it's really, really not.
