Step 3: Your paperwork needs to be prepared.

Step 10: Get a formal name change.

Step 11: You should change your documents to reflect your married name.
You can change your name on official documents such as your Ontario Health Card, Ontario driver's license, vehicle registration, passport, Social Insurance Number, and tax documents.If you want to change your name on your other documents, use the revised versions of your driver's license and Ontario Health Card first.
