If you haven't applied for an allowance yet, you need to submit this form so the military can officially release your payment.

You have to submit this form every time you apply for an allowance.The military could change your pay record.

You have to report any changes to the military so that they can adjust your pay.These may be the following instances.

The form is not long.You have to submit three copies of the form.You need to give all the information that is asked in this form.Nonpayment of your BAQ and/or VHA is possible if you don't reveal any information.

10.The details about your dependents or the people you are sharing your quarters with are provided here.You can input their name, address, and date of birth.

11.You must certify that you will provide adequate support for dependents in this section.TheINITIALS should be written in the boxes provided.

There are 12.In this section, you are asked to provide information about your monthly expenditure and the monthly expenditures of your dependents.

13The box has your signature in it.Your signature is important.If you don't sign this form, you will not be paid your bah on time.There are 14.You can input the date you filled out the form.

15.Do not input anything into the Certifying Officer's signature.The certifying officer is the one who will input his or her signature on the form.

Form 5960 can be submitted to your unit's personnel actions office or human resources office.The receiving personnel will submit the documents to the DFAS.
