My Health Pays is a rewards program for my health pays.
Taking good care of yourself will help you earn money through the rewards program.You will make money when you complete screenings or care visits.
If you earn money, it will be added to your new card.After you complete your first screening or care visit, you'll receive your card by mail.
After you use your Visa card, keep it.It will be added to your card when you earn more rewards.
For a full year, your rewards are good.After 90 days after the end of insurance coverage, funds are no longer valid.
There are rewards for completing an HbA1c test.
You must have a follow-up visit with your primary provider within 30 days of being released from the hospital.
Your reward will be determined by the type of screening your doctor recommends.One test per year will be rewarded.There are various screening types.
A few questions about your health history.You can complete this assessment by calling your health plan.The hours are 8 a.m.Mondays through Fridays are to 8p.m.You may be asked to leave a message after hours.Within the next day, your call will be returned.
A license from Visa U.S.A. Inc. allows The Bancorp Bank to issue the My Health Pays Visa Prepaid Card.The bank is a member of the FDIC.
The card is limited to certain products and services.Up to the amount of your card balance, eligible items will be covered.Remaining balance will remain on your card.You can use it for future purchases.If Visa debit cards are accepted, this card will not be used.For complete usage restrictions, see the Cardholder Agreement.
